By accessing our website or using any services provided by Echoes of Grain Home Care ("Company," "we," "us," or "our"), you agree to be bound by these Terms of Service and all applicable laws and regulations.

If you do not agree with any of these terms, you are prohibited from using or accessing our services. These terms apply to all visitors, clients, and others who access or use our services.

Echoes of Grain Home Care provides home health care and non-emergency medical transportation (NEMT) services in Ohio. Our services include, but are not limited to:

• Skilled nursing and personal care services delivered in clients' homes

• Non-emergency medical transportation for individuals requiring rides to medical appointments

• Care coordination, care planning, and ongoing support services

Services are subject to availability, geographic limitations, and eligibility requirements. We reserve the right to modify, suspend, or discontinue any service at any time with reasonable notice.

As a client or authorized representative, you agree to:

• Provide accurate, complete, and current information when requesting services

• Notify us promptly of any changes to your health status, insurance, or contact information

• Treat our caregivers and drivers with dignity and respect

• Maintain a safe and accessible environment for our staff to perform their duties

• Provide timely cancellation notice (minimum 24 hours) for scheduled services when possible

• Pay all applicable fees in accordance with your service agreement

Payment terms, rates, and billing procedures are established in your individual service agreement. General terms include:

• Services billed to applicable insurance, Medicaid, or private pay as outlined in your agreement

• Co-payments and out-of-pocket costs are your responsibility

• Invoices are due within 30 days of service unless otherwise agreed

• Late payments may incur additional fees as specified in your service agreement

• We reserve the right to suspend services for non-payment after reasonable notice

We understand that plans change. Our cancellation policy is:

• Scheduled services must be cancelled at least 24 hours in advance to avoid a cancellation fee

• Same-day cancellations may be subject to a fee as specified in your service agreement

• Repeated no-shows or last-minute cancellations may affect your ability to schedule future services

• Emergency cancellations due to hospitalization or medical emergency are exempt from cancellation fees with appropriate documentation

To the maximum extent permitted by law, Echoes of Grain Home Care shall not be liable for:

• Indirect, incidental, special, or consequential damages arising from the use of our services

• Delays or failures in service caused by circumstances beyond our reasonable control (force majeure)

• Damages resulting from inaccurate or incomplete information provided by clients

Our total liability for any claim arising from our services shall not exceed the amount paid for the specific service giving rise to the claim.

Nothing in these terms limits our liability for death, personal injury caused by negligence, or any liability that cannot be excluded by law.

All content on this website — including text, graphics, logos, images, and software — is the property of Echoes of Grain Home Care and is protected by applicable copyright and intellectual property laws.

You may not reproduce, distribute, modify, or create derivative works from our content without our express written permission.

Your use of our services is also governed by our Privacy Policy and HIPAA Notice of Privacy Practices, both of which are incorporated into these Terms by reference. Please review these documents carefully.

These Terms of Service are governed by and construed in accordance with the laws of the State of Ohio, without regard to its conflict of law provisions.

Any disputes arising under these terms shall be subject to the exclusive jurisdiction of the state and federal courts located in Franklin County, Ohio.

We reserve the right to modify these Terms of Service at any time. We will notify clients of material changes through email or a prominent notice on our website.

Continued use of our services following notification of changes constitutes acceptance of the revised terms.
